Total T, Free T, and SHBG: The Three Numbers Men Should Understand

Three lab values, one picture. What each one measures, why the total can mislead on its own, and how a physician reads them together.

If you have ever looked at your own lab results, you have probably searched for one line, the one that says testosterone, read whether it was high or low, and stopped there. It is a reasonable instinct. It is also why so many men leave with the wrong impression of their own physiology.

Testosterone is not really one number. It is at least three, and they only make sense in relation to each other. Here is what each one is, in plain terms, and why a physician reads them as a set rather than a headline.

What men are hearing

The shorthand “your testosterone” usually points at a single value called total testosterone. It gets treated like a thermostat reading: one number, high or low, done. That shorthand is convenient, and it quietly drops the two values that tell you what the first one actually means. When men feel that their results don’t match how they feel, this is very often the gap.

Total testosterone: what it is, and what it isn’t

Total testosterone measures all the testosterone circulating in your blood. Every molecule, counted together, reported as a single concentration, usually in nanograms per deciliter.

That makes it a real and useful measurement. It also makes it a sum, and sums are easy to misread. Most of the testosterone in that total is not floating free and available to your tissues. It is bound to proteins. So the total can be perfectly respectable while the usable portion is not, the way a paycheck can look fine on paper before you notice how much was already committed before it reached you. Total testosterone tells you how much is in the account. It does not, by itself, tell you how much you can spend.

SHBG: the carrier that changes the story

Sex hormone-binding globulin, or SHBG, is the protein that does most of that binding. Testosterone attached to SHBG is held tightly and is largely unavailable to your tissues moment to moment. SHBG, in effect, decides how much of your total is locked up versus in circulation.

This is why two men with an identical total testosterone can be in genuinely different situations. The man with high SHBG has more of his testosterone tied down, so less is available, even though the total reads the same. SHBG tends to rise with age and with certain conditions and medications, and fall with others. Skip it, and the total floats free of context. Measure it, and the total finally has a meaning.

Free and bioavailable testosterone: the fraction that acts

Free testosterone is the portion bound to nothing, immediately available to your tissues. Bioavailable testosterone is a slightly larger slice: the free portion plus testosterone loosely attached to albumin, which the body can still draw on. Between them, they describe what is actually reaching the system rather than what is sitting in storage.

Free testosterone is often calculated from the total, the SHBG, and albumin rather than measured directly, and the calculation has limits worth a physician’s judgment. Directly measured free testosterone exists but the method matters; not every laboratory’s version is equally reliable. This is one of several places where reading labs well is a clinical skill, not a lookup. But the concept is simple: this is the number that most closely tracks what your body has to work with, and it is frequently the one that explains a “normal” total in a man who does not feel normal.

How a physician reads the three together

No single one of these values is the answer. Read as a set, they start to tell a coherent story.

A solid total with a high SHBG and a low free level points one direction: plenty of testosterone on paper, less of it usable, which can fit a man with real symptoms and a reassuring-looking report. A low total with a low SHBG is a different picture and a different conversation. A perfectly ordinary set of three, in a man whose symptoms turn out to track better with sleep, stress, alcohol, thyroid, or another cause entirely, is a genuinely valuable result, because it redirects attention to where the problem actually is.

Good interpretation also folds in the things that don’t appear on the panel at all: your symptoms, your age, your history, the time of day the blood was drawn, and how these numbers compare to where you were years ago. Guidelines are clear that diagnosis rests on symptoms together with unequivocally low, properly timed measurements, repeated to confirm, not on a single value read in isolation.

You do not need to become your own endocrinologist to benefit from understanding this. You need to know enough to ask the right question. When someone tells you your testosterone is “fine,” the useful follow-up is simple: fine according to which of the three numbers, and read against what? A practice worth your time should be able to answer that without making you feel small for asking.
